Transcript Form - Directions for Completion

Before beginning, please review the following documents attached to this form.

Processing Timeline

Transcripts are processed the final Monday of the end of each month.

After processing, you will receive an email from the Office of Catholic Schools that includes:

  • Your processed transcript, and
  • Next-step instructions for you.

Documents to Gather BEFORE You Begin Your Transcript Form

Before starting your Transcript Form, please gather and organize the following documents. Having everything ready will make the process much smoother and help ensure your form is processed without delay.

  1. Kansas Educator License
  • A copy of your current State of Kansas Educator License 
  • Review the RENEWAL REQUIREMENTS section of your license to understand how many points/college credit is needed for your renewal
  • Only Professional development/college credit earning DURING the EFFECTIVE and EXPIRATION dates listed on your license count for renewal. Do NOT submit PD or college credit outside those dates.
  1. College or University Transcripts (if applicable)
  • Official or unofficial transcripts showing any college credit you are using toward renewal
  1. Inservice Education Verification Forms, Certificates of Completion and/or Supporting Documentation
  • All Verification of Inservice Education forms for PD completed during your current licensure period
  • Organize them in chronological order (most recent on top).
  1. Electronic Files (you will upload TWO documents)
  1. Digital copy of your Kansas Educator License
  2. All verification forms
    • Ensure all documentation is scanned, clearly readable, and merged into one PDF file (college credit first, then all inservice in CHRONOLOGICAL ORDER from most recent to oldest).
      • File should be named in this format:  LastName_FirstName_TranscriptDocumentation.pdf (EXAMPLE: Doe_Jane_TranscriptDocumentation.pdf)

Name Entry
  • Enter your first, middle, and last name exactly as printed on your State of Kansas Educator License.
  • If your name has changed during your current licensure period, enter your current last name, and include your former name (as printed on your license) in parentheses.
    • EXAMPLE: Jane Doe (Smith)

In the table below enter the title of each inservice education activity, select the type of education activity, enter the number of credit hours or points earned, and enter the date for each activity. You must have at least the combined total points indicated on your license for renewal to submit the document.

College Credit, if applicable [ENTER FIRST]
  • College credit must be listed first, at the top of your entries.
  • Use the FULL Name of the college/university
  • Use this format:
    Institution name, Course number, Course title exactly as it appears on your transcript
    • EXAMPLE: Baker University, EDU6543, Dyslexia Strategies
Inservice Education Activities
  • After college credit, list all other professional development or service activities.
  • Click “+ Add” to create additional entries.
  • MUST List activities in chronological order, with the most recent first.
  • For each activity, complete:
    • Title of Activity
    • Type of Activity:
      • C/PL = Content or Professional Learning
      • SP = Service to the Profession
    • Points Earned (credit hours or PD points)
    • Date Completed

The form will automatically calculate your total points.

Uploading Documentation
  • Combine all verification forms and certificates into one single PDF in chronological order:
    • Most recent documentation first
    • College transcripts first, followed by all other inservice documentation
  • If your file exceeds the upload limit, email the remaining documentation to transcripts@archkckcs.org.

Saving and Submitting
  • You may save a draft at any time and return later.
  • Do NOT submit your transcript form until:
    • You have entered all college credit and PD points needed for license renewal.
    • All documentation is uploaded.
  • Once ready, click Submit to send your transcript request to the Office of Catholic Schools.

Once you click Submit, your form is considered final.
